Treatment Planning

S.M.A.R.T.

Treatment plans established with participants at Detour Transitions' outpatient treatment program are geared to meet 5 criteria. Our treatment counselors are trained to ensure that each goal, each action step is as follows:


S - Specific

Each treatment plan consists of goals and objectives tailored to the aspirations of the participant. 


M - Measurable

Goals are accompanied by various action steps taken both by the participant and the counselor. These goals allow a participant to see the progress that they are making towards their specific goals.


A - Achievable

Success in reaching goals is one of the empowering aspects to success treatment and treatment planning. With each participant the outline of the treatment plan is adapted as participants move closer towards, reach and ultimately surpass their various goals.


R - Relevant 

When participants build their treatment plan with their counselor their input is vital. Personalization of a treatment plan is a key to the success of the stated goals. This helps participants to feel empowered to succeed at the goals outlined. Goals must be important to the participant.


T - Time-bound 

At the time when a participant creates their treatment plan, they are asked, "How long do you think this will take you" by their counselor. This time frame is placed in the plan and during each session, both group and individual, counselors will inquire on progress towards these goals. Treatment plans are reviewed and often updated every 30 days to ensure that participants do not become stagnant when working towards longer term goals. This attempts to motivate each participant to stay in the action stage of change.
